Good Faith Estimate

What Is a Good Faith Estimate?

A Good Faith Estimate outlines the expected charges for your planned healthcare services. It helps you understand anticipated costs before receiving care. It is not a bill.

Who Is Eligible?

You may qualify for a Good Faith Estimate if you do not have health insurance or choose not to use your insurance to pay for the services

What Does the Estimate Include?

Your estimate may include office visits, lab tests, imaging, procedures, and related fees or items. Final costs may vary if additional services are needed.

What If Your Bill Is Higher Than Your Estimate?

If your bill is at least $400 more than your Good Faith Estimate, you may be eligible to dispute it under federal rules. You generally must start the process within 120 days of receiving the bill.

Keep Your Estimate for Your Records

Keep a copy of your estimate and contact Opsam Health if you have questions.
