What To Bring To Your First Appointment
Documents
- A photo ID (like your driver’s license or passport)
- Health Insurance card, Medicare and/or Med-Cal card(s), if you have them)
For Self-Pay
If you don’t have health insurance, please bring proof of income to apply for out sliding fee program. It will be used to help you enroll in a sliding fee scale.
You can used things like pay stubs, tax returns, or a bank statement.
If you need help getting insurance, we can assist you with that too.
If you need assistance in applying for insurance, we can help you through our Certified Enrollment Counselor.
