Enhanced Care Management

Enhanced Care Management (ECM) is a service specifically designed to provide extra support for eligible Medi-Cal patients with complex health and social needs. A dedicated care team helps coordinate medical appointments, social services, and other essential resources to ensure patients receive the comprehensive care they require. The primary goal is to enhance the overall health and quality of life for these patients through personalized, continuous support, ultimately aiming to reduce unnecessary hospital visits.

Chronic Care Management

Chronic Care Management helps eligible Medicare patients with long-term health conditions, such as diabetes or heart disease, effectively manage their care. This service encompasses creating a personalized care plan, conducting regular check-ins with a dedicated healthcare team, and ensuring medications and treatments are working as intended. The overarching goal is to maintain the health and well-being of these patients and minimize unnecessary hospital visits.

Remote Patient Monitoring

Remote Patient Monitoring (RPM) allows doctors to keep track of a patient’s health from home using special devices, like blood pressure monitors or glucose meters. These devices send information directly to the healthcare team, so they can see how the patient is doing and make any needed changes to their care. It helps catch problems early and reduces the need for in-person visits.
